SPHERE questionnaire

The 34-item Somatic and Psychological HEalth REport (SPHERE-34), assesses a wide range of somatic and psychological symptoms. The instrument was derived for use in primary care and other general medical settings where somatic symptoms often overlap with psychological complaints. A short form of the SPHERE-34 was developed (SPHERE-12) for classifying common mental disorders. The SPHERE-12 includes two subscales (each consisting of six items) that best describe persistent fatigue ('SOMA-6') and the common forms of depression and/or anxiety ('PSYCH-6'). The SPHERE-12 subscales create two levels and three types of mental disorder: Level 1 classifies caseness for both the 'PSYCH-6' (score >=2) and 'SOMA-6' (score>=3) subscales (Type 1); Level 2 classifies caseness for either 'PSYCH-6' only (Type 2) or 'SOMA-6' only (Type 3).
